At FutureCare, our Regional Care Managers (Hospital Liaisons) are an integral part of the admission process, serving as the liaison between the hospitals and our facilities.  The Regional Care Manager is responsible for completing the pre-admission screening of patients for appropriateness to FutureCare by reviewing the clinical record, and talking with discharge planners to determine if there is a skilled need. The care manager helps to educate the hospitals to the programs and services we can provide at our various locations. The Regional Care Manager works on-site at the hospital, reviewing referrals, communicating with the discharge planners and other key clinical personnel, the patient and family, and our facility admission departments.  The Regional Care Manager is also responsible for building new business from within their hospitals through ongoing business development.  We offer a Competitive Salary, Excellent Benefits Package, Tuition Reimbursement, Employee Assistance, and a 401K Plan.   ***Salary Range $88,000 - $97,000 Full Time Annual*** Salary Disclosure StatementThe salary mentioned above reflects the potential base pay range for this role.
