Regional Care Manager (Hospital Liason)

At FutureCare, our Regional Care Managers (Hospital Liaisons) are an integral part of the admission process, serving as the liaison between the hospitals and our facilities.  The Regional Care Manager is responsible for completing the pre-admission screening of patients for appropriateness to FutureCare by reviewing the clinical record, and talking with discharge planners to determine if there is a skilled need. The care manager helps to educate the hospitals to the programs and services we can provide at our various locations.
